The below button will direct you to our online short referral form. The form will take no longer than a few minutes to complete. Once received, a member of staff will then contact you to arrange a 'Meet and Greet' also known as an initial assessment, to ensure we are the right service for you.

Once your form has been received, your local Area Manager will contact you for an in-person 'Meet and Greet', or via Zoom. This meeting will be for us to gain in-depth information to ensure that we can support the individual’s needs and to give you more information and answer any questions that you may have.

This initial assessment 'Meet and Greet' can take place at the Participant’s home, online or another private setting provided by the Participant orParent/Guardian. Or it can take place in a private office setting that will be organised by the local Area Manager, whichever is best and most comfortable
for you.

With your consent, there will be an audio recording created of this meeting to ensure that all of information is captured. This recording will solely be used for the purpose of the Area Manager creating a personalised Care Plan. This recording will be deleted within 7 days of the meeting taking place.

Once the Area Manager has created the Care Plan, a copy will be securely emailed to the Guardian of the Participant to verify that the information is correct, and to sign The Inclusion Project’s Terms and Conditions. This will all be completed electronically.

Your Area Manager will then arrange for a free Taster Session for the Participant. The Participant will then be able to access all available sessions with The Inclusion Project. All bookings will be managed by your local Area Manager, and bookings can be made through our website or by email to your Area Manager.
